Attention LCC Parents and Families:
The Pre-participation Physical Form for the upcoming school year is available.
Students in grades 7–12 who plan to participate in athletic activities must have a new physical examination record on file before participating.
In addition, all incoming 7th-grade students are required to have a new physical record on file before the start of the school year, regardless of whether they plan to participate in athletics.
Families are encouraged to schedule physical appointments early and bring the attached form to the appointment. Forms can be accessed using the link provided or may be downloaded from the school district website (www.lccschool.org). Copies may also be picked up at the High School and Middle School offices.
